Summer DYCD Registration

Step 1: DYCD Application

Fill out the DYCD application on the DYCD Portal. This is a link to the NYC portal for DYCD-funded programs. Please use this PDF to guide you when filling out the application on the DYCD website to ensure your application is filled out correctly.

Please note: We cannot accept children who deny consent to emergency medical treatment. These situations are rare, and we will do our best to contact you if such a situation arises. If we cannot reach you, we must have your consent to allow us to treat your child and keep them safe.

Step 2: DocuSign Consents

Fill out the DocuSign, and sign the required consents by clicking here.

When you complete the DocuSign, please ensure the following:

On the first DocuSign screen (before the form opens): Enter your own full legal name in the “Name” box and your own email in the “Email” box.
Inside the form itself: Make sure to enter your daughter’s information only in the fields marked "for the student."

How do I know if my child was accepted into the program?

Once all your documents are complete and your application has been processed, you will receive an email from DYCD informing you of your child’s status in our program.
